Privacy Policy Statement

Ammed Cancer Center (Central) Limited ("the Company") is committed to protecting your privacy.

Types of Data Collected
From time to time, it is or will be necessary for you to supply to the Company data such as your name, insurance account number, contact number, postal address, email address, HKID /passport number, birth date, gender, age, credit card number, bank account number, occupation, education, and personal interests etc.. This website may also place 'cookie' in your computer browser to collect information which may include, but not limited to, relevant login and authentication details as well as information relating to your activities and preferences across this website.

Failure to supply such data may result in the Company's being unable to realize your preferences or provide its services to you.

Purposes for which data are used

The purposes for which we may use the data collected or to be collected by the Company are divided into obligatory purposes and voluntary purposes. If the data is to be used for an obligatory purpose, you MUST provide the data to the Company if you want the Company to provide the services for which you are applying. If the data is only to be used for a voluntary purpose, the Company will obtain your consent and you can tell the Company not to use the data for that purpose and we will not do so.

Purposes for which it is obligatory for you to provide the data are:-
(1)   Verification of your identity;
(2)   Providing you with suitable medical treatment and related services;
(3)   Processing billing and payment from you/ your employers and insurers (if applicable);
(4)   Collecting any outstanding amounts from you;
(5)   Handling and following up enquiries and complaints made by you;
(6)   Meeting the requirements to make disclosure under any applicable law;
(7)   To process your requests or enquiries;
(8)   Do send you administrative communications, such as information about future changes to this Privacy Policy Statement;
(9)   For our internal business and administrative purposes; to assist in law enforcement purposes and to meet requirements imposed by law or for claims-related purposes;
(10) to determine amounts owed to or by you;
(11) to enforce your obligations, including without limitation the collection of amounts outstanding from you and those providing security for you;
(12) purposes relating thereto.

Purposes for which it is only voluntary for you to provide the data are:-
1. sending to you direct marketing, promotional information and/or materials and/or offers and news of the Company's  products and services,
2. sending you our future marketing purposes and/or joint promotion with our subsidiaries, holding companies, associated companies, affiliates or in conjunction with products/ services offered by our affiliates or business partners; and
3. conducting customer and service surveys.

Notice for Direct Marketing
The Company intends to use your personal data including your name, address, telephone numbers, email and other contact information provided above for marketing communications such as direct-mail, email, telephone calls and SMS containing news, promotions, offers, and events to be offered by us or by our business partners or future marketing purposes and/or joint promotion with our subsidiaries, holding companies, associated companies, affiliates or in conjunction with products/services offered by our affiliates or business partners in relation to the following classes of products and services: service programmes (traditional Chinese medicine, dental services & medical specialties), seminars/health talks, clinic activities and events, newsletters/other publications, health awareness campaigns and events, social networking, media and public relations.

Nevertheless, we may not use your personal information for direct marketing without your consent. If you do not allow and agree us to use the data for the voluntary purposes including direct marketing as listed above, please tick the opt-out checkbox when you fill in the Company’s forms and any other documents (if applicable).

We also provide opt-out channels for you to unsubscribe from receiving our information and service. Please inform us by mail or email below with your name, membership number and class(es) of products, services and/or subjects that you would like to opt-out:-

We are committed to preserve high confidentiality of your personal data collected. All data will only be used exclusively for the Company, and will not be sold, traded or rented in any forms through any means to any other parties.

Disclosure of Personal Data
Data held by the Company relating to you will be kept confidential. In cases where the Company do collect the data from you, we will:

1. tell you (by way of this Privacy Policy Statement or by a separate notification) that the Company is doing so and the use that the Company will make of such data we collect;

2. where relevant, give you the opportunity to “opt out” (that is to restrict the uses the Company will make of such data); and

3. tell you how the Company will store the data and how you can review, change and delete the data the Company has stored.

The Company may, where such disclosure is necessary to satisfy the purpose, or a directly related purpose, for which the data was collected provide such data to the following parties:

(1) Any of the medical centres operated by us and staff (including but not limited to nurses, doctors, dentists,TCM practitioners, pharmacists, and other health care professionals) who provide services to you at such medical centres;

(2) Any agents, contractors, or third party service providers if we need their services (such as clinical, laboratory, or technical services) in connection with services provided to you;

(3) Other nurses, doctors, dentists, TCM practitioners, pharmacists, or health care professionals, in the case of referrals from or to any of such professionals;

(4) Your employers and any insurers who are providing medical benefits to you (if necessary);

(5) Any debt collection agencies (if necessary);

(6) Any person or entity to whom we are obliged to make disclosure under the requirements of law; and

(7) Any government and judicial bodies or regulators.

Access and Correction of Personal Data

You have the right to:
1. check whether the Company holds any personal data relating to you and the right of access to such data;
2. require the Company to correct any data relating to you which is inaccurate; and
3. ascertain the Company's policies and practices in relation to personal data.

(5) 任何收賬代理(如必要);
(6) 根據對本公司有約束力之任何法例或條例規定,本公司有義務向其披露有關資料之任何人士或實體;以及
